CONGRESS MEMBERS
Registration fee entitles the delegate to attend all Scientific and Poster Sessions,
Opening Ceremony, Welcome Cocktail, Industrial Exhibition, and to receive coffee break tickets and the congress kit (including Abstract Book).
Pre-registration deadline has expired. Only onsite registration is possible, starting July 15 at the SECC Congress Centre. You will find here all information on onsite registration
DELIVERY
• You can pre-register online or send/fax the Pre-Registration form to: ERA-EDTA Congress Office, Via Spolverini 2, 43100 Parma, Italy, Fax: +39-0521-959242.
• Make sure that you don’t send your form to the local housing bureau or to the President/Secretary’s Offices in UK.
• Do not forget to send the Pre-Registration form completely filled in, when you send the payment by bank transfer: without it we cannot insert you in the pre-registered delegates’ list.
• In order to benefit of the Pre-Registration fees, forms and payments, all must be received within the deadlines. Submission of form without payment within deadlines, does not entitle to benefit of the reduced fees later.
• The mentioned deadlines are a receipt and not a postal one.
PAYMENT
• Pre-Registration forms received without payment will not be processed.
• Please clearly indicate your name, address and ERA-EDTA on money orders.
• If you send your Pre-Registration form by fax with a credit card payment, do not also mail a copy, since it may result in a duplicate registration and charged twice. We don’t accept Visa Electron, American Express or Diners card.

Please indicate “free of charge for the beneficiary account” and send a copy of the bank transfer via fax or e-mail together with your form. REQUEST OF PAYMENT: if you are in need of an official request of payment in order to transfer the registration fees, you must contact the ERA-EDTA Congress office well in advance before the set deadlines.
MEMBERSHIP
• Membership and payment of ERA-EDTA membership fees will be verified for all individuals who indicate they are ERA-EDTA members, thus make sure that you are a current ERA-EDTA member and have regularly paid your membership fees.
• If you wish to pre-register as member and you are not one, contact the ERA-EDTA Membership Office to become a member before preregistering.

Confirmation letter and congress materials will not be sent to congress members with outstanding membership fees or status.
CONFIRMATION NEW!!!
A confirmation letter and congress materials (badge, bag voucher and coffee tickets) will be sent by post to regularly pre-registered congress members after the early deadline for those who have pre-registered by March 6 and after the late deadline for those who have pre-registered by June 5.
Confirmation will also be sent by e-mail provided that we have received it.
If you do not receive your badge within June 30 2006, please contact the ERA-EDTA Congress Office immedately: mailing delivery problems may happen. After June 30 you can pick up your materials at the Pre-registration Reprint Desk starting July 15.
Replacement badges are subject to a fee of euro 5 each.

HOW TO RECEIVE UEMS CME CREDITS
ERA-EDTA will track attendance of Congress members during the congress in the SECC each day.
As regularly registered congress members enter or exit the SECC each day, hostesses at the CME credit desk will read the bar code printed on the congress member’s badge. Congress members will then be awarded the number of credits towards the hours actually spent participating in the scientific sessions of the congress.
Regularly registered congress members will then receive the CME UEMS certificate directly at their personal (home or work) address after the congress from ERA-EDTA.
RECEIPTS
• Receipts or written confirmations of payment are not automatically sent. If you wish to receive a receipt of the payment, tick the receipt box in the form.
• Due to the fact that ERA-EDTA is a registered English Charity, invoices cannot be issued.
BLANK GROUP REGISTRATIONS
(companies only)
Companies that wish to pre-register a group of more than 15 delegates with the Blank List method, must contact the ERA-EDTA Congress Office for all details. Deadline for bookings and payments must be done by March 6 2006.
After this date, normal pre-registration procedure is applied to all, groups included.

CANCELLATION POLICY
Before June 5, 2006 75% of the amount paid will be reimbursed
After June 5, 2006 no refund of the amount paid
All cancellations must be done in writing and must be received (not sent) within June 5, 2006.
• All refunds, if any, will be done after the Congress and will be in euro currency only. In order to benefit of the refund, all congress materials must be returned to the ERA-EDTA Congress Office.
If the registration must be cancelled for any reason (even at the last minute), it cannot be transferred or taken by another person.
HOW TO GET THE CERTIFICATE OF ATTENDANCE
All regularly registered congress members can collect their certficate of attendance at the Attendance desk located in the Registration Hall starting July 16 after 15.00 hours.
To obtain your certificate of attendance, you must go to the Attendance Desk and hand in your badge. You will receive a personalized certificate that will state exactly the days of your participation in the congress.
ON-SITE CONGRESS MEMBERSHIPNEW!!!
(please read this carefully after June 5, 2006)
On-site registration will be made by means of self service data input. Each participant must fill in the on-site registration form in the computer with information requested, at the New Registration Desk (starting July 15, from 8.00 to 18.00 hours), located in the Registration Hall of the SECC in Glasgow. Information such as name, full address, e-mail and fax will be mandatory for each registration, groups included.
It will be possible to pay by bank cheque (we don’t accept personal cheques), bank transfer (in which case a copy of the original bank transfer receipt issued by the bank must be handed in while registering), cash (euro only) and credit card (Visa and Eurocard/Mastercard). We don’t accept Visa Electron, American Express or Diners card.

file, even though the payment has been sent.
If you have paid by bank transfer the late fee after the final deadline of June 5, you must pay the difference towards the on-site fee and give in both receipts on-site in Glasgow. Or you can pay the difference on-site directly.
On-site registration does not guarantee complete congress bag.
*available only on-site: to qualify for this fee, you must be 35 years or under and you must personally hand in a photocopy of your identifi cation picture document, show your original student’s card and give a signed official letter from your mentor/university to proof your status. ERA-EDTA reserves the right to verify all declarations.
Companies or other subjects will not be allowed to register delegates at this fee.
**Exceptionally low registration fee (Euro 50) for students living in the country where the congress is held (United Kingdom), under the age of 28.
This special fee is available only onsite: to qualify for this fee, you must be 28 years or younger and you must personally hand in a photocopy of your picture ID, show your original British student’s card and give a signed official letter from your British mentor/university to prove your status.
ERA-EDTA reserves the right to verify all declarations, even after the congress.
Companies or other subjects will not be allowed to register delegates at this fee.
1 Day fee entitles admission to the session and poster halls only on the date mentioned on the congress badge and to receive the proper UEMS Credits and Certificate of Attendance
for that same day. It is possible to purchase a maximum of 2 single days.
